Situation
An elderly woman in her early eighties had been discharged from hospital following a hip procedure. She had three children, all working, and a schedule of follow-up appointments across two different specialists.
Approach
We coordinated all follow-up appointments, managed transport, and attended consultations to take notes and relay relevant information to the family. Medical records were kept organised and accessible.
Outcome
The family described the post-hospitalisation period as significantly less stressful than expected. No appointments were missed, and the family felt genuinely informed at each stage of recovery.
